light. Gram-positive bacilli, beaded. Suspect mycobacteria:
initiate additional testing (see Fig 2).


Mycobacterium kansasii

light microscopy, MPV. Purulence, moderate.
Acid-fast bacilli, numerous. Mycobacterial cultures grew
Mycobacterium kansasii.

microscopy, MPV. Amorphous debris, heavy. Bacillary shapes,
negative image. Oil removed from smear, decolorized with
acid alcohol, and immediately restained with Ziehl-Neelsen
acid-fast stain. Acid-fast bacilli, numerous. Specimen
recovered; acid-fast culture requested by laboratory.
Suspicious for tuberculosis.

Mycobacterium tuberculosis

Typical acid-fast bacteria, numerous. Impression:
mycobacterial disease.
The patient had been placed in respiratory isolation
following physical examination and history and was
immediately begun on antituberculosis therapy following
receipt of the direct examination report. Mycobacterium
tuberculosis
was identified from culture.

Rhodococcus equi.

Amorphous debris, moderate. Gram-positive bacilli, medium,
beaded. Follow-up acid-fast stain positive. Chest radiograph
with right upper lobe mass. Culture isolation of Rhodococcus
equi.

Actinomyces israelii.
Fig 6.  Blood culture, sediment smear, Gram stain, light
microscopy, MPV. Blood. Gram-positive bacilli, branched,
beaded. Morphology consistent with Actinomyces or
Propionibacterium spp. Culture isolation of Actinomyces
israelii.

Nocardia
Fig 7.  Expectorated sputum, smear, Gram stain, light
microscopy, MPV. Purulence, light. Local materials, light.
Amorphous debris, moderate. Gram-positive bacilli,
branched, beaded. Morphotype consistent with Nocardia or
Actinomyces
.

Nocardia spp
Fig 8.  Expectorated sputum, smear, partial acid-fast
stain, light microscopy, MPV. Partially acid-fast bacilli,
branched, beaded. Morphology consistent with Nocardia
spp. Impression: nocardiosis.
